Job Description

Business Context and Main Purpose of the Job
Providing medical and anaesthetic care to inpatient and outpatient at Unilever Tea Tanzania Limited – medical and occupational health department.

Main Accountabilities
• Diagnosis and treating the OPD patients including ordering laboratory investigation, drugs, prescription, counselling, admission of those patients with severe illness, performing minor surgeries e.g., suturing wounds, speculum examination, reduce fractures, emergency oral care procedures and assist in major operation.
• Responsible for ward rounds for in patients and record keeping in different register, check status and hygiene of patients.

Organizational context and scope
Under the overall supervision of the chief of mission (com) and the direct supervision of the migration health officer (mho), the successful candidate will be responsible for the carrying out the following duties and responsibilities in relation to the migration health assessment centre (mhac) in kasulu/makere, tanzania.

Responsibilities and accountabilities
1. Ensure the efficient daily operations of the migration health assessment center (mhac), in close coordination with the chief migration health officer (cmho).
2. Conduct the kasulu/makere,mhac’s migration health assessment process to fulfil the technical requirements of the resettlement countries in the areas of:
3. Medical examinations;
4. Imaging;
5. Laboratory testing;
6. Vaccinations;
7. Tb management;
8. Treatment and referrals;
9. Pre-departure procedures and medical movements;
10. Documentation, certification and information transmission; and,
11. Other technical areas as may be required
12. Ensure proper identification of refugees and migrants during the health assessment and record all relevant health information in standard forms; ensuring completeness and accuracy of the recorded information.
13. Perform treatment for tb and sexually transmitted infections and provide support to the hiv and counselling activities. Oversee and coordinate the management of tb cases to ensure effective tb treatment.
14. Oversee and coordinate accurate and effective provision of immunization and presumptive treatment programmes in full compliance with the technical guidelines and protocols of the resettlement countries. Assist cmho in monitoring, supervising and educating all staff in the delivery of these programmes.
15. Contribute to and maintain a system of quality improvement for each service area within the mhac. Undertake quality control activities on a regular basis, including practice observation, desk audits and use of selfassessment tools. Use data analysis and web reporting system to monitor performance indicators. Ensure implementation of the global iom standard operating procedures (sops); create and implement kasulu/makere specific sops for each service area. Ensure proper reporting and management of incidents according to the guidance note for incident management.
16. Organize systematic collection, processing and analyses of migration health data according to guidelines established by the cmho. Ensure data quality. Provide periodic, as well as ad-hoc reporting to the cmho for migration health activities.
17. Ensure that all data related to heath assessment programmes is appropriately entered to migrant management operational system application (mimosa) and other related databases.
18. Oversee the financial aspects of the mhac in close coordination with the mission’s finance staff: supervise budget preparation, suggest adjustments and cost-effective solutions, and review financial reports.
19. Provide oversight and coordinate the procurement of medical equipment, vaccines, medications and other medical supplies in coordination with the cmho and the resource management unit.
20. Perform such other duties as may be assigned by the supervisor.

Required qualifications and experience
Education

• university degree in medicine from an accredited academic institution with four years of relevant experience;

Experience

• minimum of four (4) years, post-graduation, (post internship in countries with mandatory internship programs) continuous clinical experience, preferably in a multidisciplinary hospital setting. The last clinical posting should be within the last five years; and,
• valid license to practice within country is mandatory.

Languages

• support the planning and execution of mobile missions if applicable.
• assist with pre-departure documents, briefings and preparations as necessary.
• perform medical escort duties when required to ensure migrants receive continued care throughout all phases of migration (before departure, during the journey and at the final destination).
• maintain collaborative relationships with various partners to facilitate the follow up and relevant information sharing. Partners include internal iom departments, other health providers, panel physicians, embassies other un agencies or ngo’s and others as applicable.
• collect statistical information and share with the senior migration health nurse or cmho as per preset and agreed upon format.

Required qualifications and experience
Education

• nursing degree with a minimum of three years relevant clinical experience.
• valid license to practice nursing in tanzania.

Experience

• excellent technical skills, including in phlebotomy.
• training or working experience in the areas of tuberculosis management, mass immunizations, communicable diseases, laboratory testing or public health is an advantage.
• computer literacy required: ms office suite (word, excel, access)

Job Description
Providing technical support in the implementation and expansion of integrated quality HIV/FP services to both key populations (KPs) and vulnerable populations (VPs), designated health facilities near hotspots, and through mobile/outreach services, index testing, partner notification, HIVST and Prep. Responsible for the overall management of clinical service provision at site level including point of care CD4 testing and contribute to the systematization and establishment of referrals/linkages with HIV/AIDS care and treatment and other services. Providing oversight in the provision of HIV counseling and testing services, TB, GBV, alcohol and drug screening and referrals, HIVST, PreP, care and treatment referrals and provision of adherence counseling for KVPs with HIV. Also collaborating with the family planning leads to ensure quality and safe delivery of family planning services at designated sites, using both short and long-acting FP methods. Act as main overseer of all clinical care related services for KVPs, including addressing the health needs of KVPs PLHIVs; and collaborating with MOHSW, care and treatment implementers, home-based care partners, and communities to support KVPs’ retention within HIV prevention and treatment continuum and connecting clinical services to KVPs at home.

Main Function/Responsibilities
• Lead the coordination, supervision and provision of HIV testing and Services (HTS), HIVST, PreP, Recency testing, family planning service provision, linkages to care and treatment support, and referrals to GBV, TB, STI and other biomedical services at mobile sites, index homes and partner notification modalities as well as support the MOHSW in selected health facilities.
• Train staff and community health workers on clinical management and integration of HIV, Prep, FP, TB screening, and STIs
• Ensure all clinical procedures at health facilities and mobile outreaches follow national guidelines and protocols developed by the MOHSW.
• Provide on-going coaching, mentoring and hands-on supervision to clinicians in mobile outreach, index testing and partner notification modalities and health facilities
• Lead the process of developing protocols and standard operating procedures for referrals and case management of HIV+ KVP.
• Ensure linkages of KVPs to other behavior and structural services which facilitate positive protective behaviors
• Provide technical assistance and participate in supportive supervision of clinical staff to ensure quality health care delivery; support the MOHSW to do the same in select sites
• Provide technical support in development and writing of concepts and proposals, for research and documentation of activities.
• Train community counselors on standards- and competency–based training in HIV, HIVST, PreP FP, community and adherence counseling, patient advocacy (including for GVB), case management and partners tracing
• Prepare monthly, quarterly and annual reports relating to curative and preventive clinical activities; assist in preparation of annual proposal and progress reports as per donor requirements.

Minimum Requirements
• MD Degree with 1-3 years of relevant experience in the provision of community-based biomedical and KVP-focused clinical services; OR Master’s degree in public health, nursing; MBA with 3 to 5 years of relevant experience; OR Bachelor’s Degree in nursing or clinical officer with 5 to 7 years of relevant experience.
• Robust clinical experience in HIV/AIDS testing and counseling, care and treatment, and family planning.
• Excellent written, presentation, communication and organizational skills in both English and Kiswahili
• Demonstrated experience in clinical training and mentoring
• The ability to liaise with senior MOHSW officials from national, regional and district level and representatives of NGOs/ CBOs.
• Demonstrated in-depth understanding of Tanzania healthcare system, particularly the public health system.

About THIS 2022-2023:
• The Tanzania HIV Impact Survey (THIS) is a national population-level HIV impact assessment (PHIA) conducted with support from the U.S. President’s Emergency Plan for AIDS Relief (PEPFAR) and technical assistance through the U.S. Centers for Disease Control and Prevention (CDC) and ICAP at Columbia University. THIS is being led by the Government of Tanzania (GoT) through Tanzania Commission for AIDS (TACAIDS) and Zanzibar AIDS Commission (ZAC), and the Ministries of Health (MoH) of Mainland and Zanzibar, National Bureau of Statistics (NBS), the Office of Chief Government Statistician (OCGS), and the President's Office Regional Administration and Local Government (PORALG). The survey will be implemented by NBS, OCGS, and ICAP in collaboration with local partners, including the National Institute of Medical Research (NIMR), the Zanzibar Institute of Health Research (ZAHRI), Public Health Laboratories, the National AIDS Control Program (NACP), and the Zanzibar Integrated HIV, Hepatitis, Tuberculosis and Leprosy Program (ZIHHTLP).

Overall Job Function:
• Reporting to the THIS Survey Coordinator, the THIS Field Supervisors will provide strategic survey implementation and monitoring support in the planning and implementation of the THIS in the regions. The Field Supervisors will build the capacity of the Survey Team Leaders and Interviewers. He or she will coordinate field work with local representatives/community members including securing permission from local community leaders for data collection in selected clusters.

Specific Responsibilities and Duties:
• Oversee and review all aspects of fieldwork, including management of team leaders and field workers, on a daily basis.
• Ensure that field teams adhere to THIS protocol and Standard Operating Procedures.
• Maintain adequate and accurate participant records, as per the study protocol.
• Determine readiness for survey implementation in each enumeration area (EA) and complete pre-implementation steps.
• Ensure that all necessary equipment is available and working properly.
• Oversee transportation of field samples to the Satellite Laboratory and/or National Reference Lab.
• Assess ongoing performance of Field Team Leaders and Nurses/Interviewers and identify training needs.
• Perform EA and Household spot checks when and where needed.
• Plan the logistics for fieldwork data collection.
• Participate in community mobilization activities, including district launches and public gatherings.
